Please use this form to document any dates you will miss lessons during this session.  Please note our cancellation and credit policy included below. 

Scheduled Absence – EquiCenter will issue up to 1 (one) credit per session that is arranged utlizing this form prior to the start of the session (due before 9/5/2026 to be considered a scheduled absence).

Medical – Missed lesson for a major medical event, such as anything requiring medical attention or a doctor’s visit.

Other Absences –  Any additional dates you will not be here this session.  You will be financially responsible for these lessons. 

Please note, we are unable to provide a make up lesson, as each rider’s slot is reserved with specific horses, instructors, and volunteer support. While we do not offer credits for common illnesses, we ask participants to stay home if experiencing symptoms to protect the health of our staff and volunteers.
