Halifax Food Hub Cooperative Member & Producer Intake Form
If you are joining the cooperative and are not a producer, please only fill out the first section of this form. 
Please read the Co-op Membership Handbook before filling out this form.
Upon approval, Halifax Food Hub will issue an invoice for your membership share and then will assist with the onboarding of your products. 
Membership shares are a one-time purchase of $50.
There is an annual fee for those selling products through the Halifax Food Hub is $50 (waived your first year). 
Payment can be sent as e-transfer to info@halifaxfoodhub.ca
