Medical Treatment Authorization: In an event of illness or injury to the above-named gymnasts, I do hereby authorize the Saultos Gymnastics Club or its agent to arrange for proper medical treatment, including, if necessary, ambulance, hospital care and consent to the administration of anesthetics and to the performance of any operation upon the above-named gymnasts if deemed necessary upon the advice of competent medical personnel. It is understood the Saultos Gymnastics Club or it agent will use the authority only after having attempted and failed to contact me or the other listed above. *

Parent/Guardian Participation Waiver: There is a risk of injury involved in training and participating in any sport. Both the Saultos Gymnastics Club and Gymnastics PEI have tried to create a safe and controlled environment for participation. Payment Information
1. Payment can be made in cash, cheque or e-transfer to saultos@gmail.com (Password: Gymnastics)
2. Registration will be confirmed ONLY with NON-REFUNDABLE $50 DEPOSIT OR FULL PAYMENT (may include post-dated cheque for term 2 or 3)
3.$25 banking charge on all returned cheques.
4.No refunds after week #2, $30.00 cancellation fee per participant will apply in addition to the non refundable insurance and equipment fee.
5. CREDITS are based on individual circumstances by the club administrator/s, medical certificate may be required.
