This form begins the nomination process for a Superuser Award. Submission is free of charge. Finalists will be contacted two weeks after submissions close.

All applications will be reviewed on the basis on their merit.

Long-form questions
The following questions are intended to uncover unique use cases, integrations and applications of OpenStack within a given organization. There are five questions to chose from, please answer only three of them.

Finalists will be asked to set up a supplemental interview with an OpenStack representative within one week of submission closing.

When evaluating responses, we are considering the following criteria:

- Demonstration of organizational transformation
- Illustration of new product, time to market or money saved with deployment
- Growth of deployment, in terms of size and number of users
- Community impact in terms of contributions, feedback, knowledge sharing, etc.
