This form is to be used by an individual who wishes to report an athletic training issue for one of the following:

1. An individual employed / volunteering / working as a certified athletic trainer without a BOC certification (non-certified athletic trainer)
2. Other licensed healthcare professional in California claiming to be a certified athletic trainer.
3. Athletic training students or athletic training student aides providing direct care to patients without supervision from a BOC-certified Athletic Trainer or other licensed healthcare professional. 
4. Harm caused to a patient under the care of a certified athletic trainer.
