CAAMT Disaster Relief Fund Member Application Form
This is a form for members of the California Association for Music Therapy (CAAMT) to utilize when applying for the Disaster Relief Fund. Members may apply for funds when they have experienced a personal or natural disaster that has interfered with their ability to serve the community as a music therapist, or when it has interfered with their music therapy practice. For distribution of funds of $600 or more, you must complete and send a W-9.

If you haven't already, please email any documentation you have of the disaster that occurred (i.e. insurance damage estimate, pictures, etc.) to info@caamt.org *
Please provide your best overall monetary estimate of your loss due to the disaster. *
For distribution of funds of $600 or more, you must complete and send a W-9. *
Please email an itemized inventory of losses (i.e. items, income, etc) related to your music therapy practice to info@caamt.org. *
Please provide a description of your music therapy practice and how CAAMT, as a music therapy centered organization, may assist you. *
