Please fill out this form as completely as possible. Know that we'll do our best to fulfill as much as you request within the parameters of your price point.

Once you place the order, we'll create and email you an invoice (including calculating shipping). Once the invoice is paid, we'll assemble your box. Fill out this form separately for EACH BOX you want to receive if you're ordering for more than one person to help us track price points and individual box preferences.

If we have fulfillment issues with any items in your box, we'll contact you right away to confirm any necessary changes or delays.

Boxes are sent once a month before the end of the month. Invoices are sent around the middle of the months for those who filled out requests before the 15th of the month.
