As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate in all aspects of cheerleading at Venom Cheer. I understand that there are certain risks of injury inherent in the practice of this sport and other related activities incidental to my child's participation, and I am willing to assume the risks on behalf of my child. In addition, to giving my full consent for my child's participation, I do hereby waive, release, and hold harmless Venom Cheer, its coaches, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation. In case of an accident, I agree to allow Venom Cheer to call 911-EMS for my child at the expense of the parents/guardians. I give permission to have my child treated in an emergency situation. I give Venom Cheer the right and permission to film or photograph my child or myself for any productions/promotional purposes associated or in any way connected with Venom Cheer. $1 will be charged BEFORE class begins.
