Thank you for your interest in Hale Charter Academy. Please complete this application to be considered for the Fall 2018 Open Enrollment lottery drawing. Computers are available in the Attendance Office for those wishing to complete the application at the school.

Only one application per student is allowed.

Siblings must apply, as sibling permits are not issued.

The process of this application is dependent upon accurate and complete information.

Transportation is NOT provided. Parents or guardians should plan to provide their own transportation.

The drawing will be held on Thursday, April 12, 2018 at 5:00 P.M. in the MPR. Selected applicants will be notified by mail or phone call and do not need to be present.

You will receive an email confirmation after you submit, please save a copy for your records.
