The Certificate of Attendance forms will be delivered to one of your classes within 48 hours of submitting this request. It is imperative that you thoroughly review all information regarding the laws and regulations for the Certificate of Attendance before completing this form.

Important: If you are deemed ineligible for the DMV Certificate of Attendance due to excessive absences or truancies, you will receive a formal notification of denial, which will also be delivered to one of your classes.
