International School of Monterey Volunteer Driver Application & Pledge
As a field trip driver, you will provide a highly valued service to ISM students. The availability of volunteer drivers allows ISM to plan exciting off-campus learning opportunities that greatly enrich the student experience. However, that enrichment cannot come at the cost of student safety.

Ensuring the safety of students, and all participants, on every trip, is paramount. Toward that end, we go through a number of steps to make sure that all field trip drivers are safe:

(2) We require that you submit this application and pledge each school year before we authorize you to drive that year. We rely on this self-report as the most important step in our driver screening process, so please provide complete and accurate information. YOU WILL NEED TO HAVE YOUR LICENSE IN HAND TO COMPLETE THE FORM. The process takes approximately 10 minutes.

(3) We cross-check self-reports against DMV records in order to ensure accuracy.

(4) You must complete a short defensive driving course. You will need to contact the school Office Manager to have the course assigned to you. It will record your completion and issue a certificate of completion. The entire process will take you 25 minutes to complete. No need to bring in your certificate, just email the Office Manager when it has been completed.

(6) ON THE DAY OF EACH FIELD TRIP, we require that you SHOW CURRENT COPIES OF DRIVER'S LICENSE, VEHICLE REGISTRATION (for the vehicle being driven that day), and PROOF OF INSURANCE. By California law, you should have these documents in your possession at all times when driving, so this requirement presents no new burden.

(7) We observe volunteer drivers during fieldtrips to note any areas of concern or inconsistency with the Volunteer Driver Pledge. If concerns arise with your driving, a member of the administration will meet with you to discuss or to communicate that your driving authorization has been suspended or revoked.

We will not authorize you to drive on fieldtrips if any of the following are true:
1) Your DMV report is substantively different than your self report.
2) You have been convicted of any vehicle-related misdemeanor or felony (e.g. DUI).
3) You choose to not provide information or to make any acknowledgment or pledge requested on this form.
4) You have been involved in more than 1 accident in the past 5 years where you were at fault.
5) You have been convicted of more than 2 moving violations in the past 5 years.

WE RESERVE THE RIGHT TO DECLINE OR WITHDRAW VOLUNTEER DRIVER AUTHORIZATION AT ANY TIME based on any factor or combination of factors that causes us to believe that students would be safer traveling with another driver. In all cases where we decline or remove authorization, we will provide you with an explanation and allow you the opportunity to provide corrected or additional information.

I understand and accept that, as a volunteer driver, the limits and coverages provided by my personal automobile insurance are applicable to any accidents or incidents that involve my vehicle while I am serving as a volunteer driver for ISM, and that any insurance secured by ISM provides only secondary coverage.

Volunteer driver pledge
As a volunteer driver for International School of Monterey, I understand that my safety and the safety of students comes first. I understand that driving on fieldtrips is a privilege, not a right. Therefore, I pledge to: (1) follow all ISM policies and procedures and any directions provided by school staff members and lead volunteers; (2) comply with all laws and regulations concerning driving, including laws pertaining to the use of seat belts, child safety seats, and cell phones; (3) drive no faster than the speed limit during fieldtrips; (4) promptly notify the fieldtrip supervisor of any physical conditions, vehicle defects, or road conditions that might affect my safety or the safety of those I am driving; (5) notify the school administration of any new vehicle accidents and convictions; (6) complete an Accident Report provided by ISM if involved in an accident during a fieldtrip, and cooperate with the police, the school administration, and ISM's insurer, insurance adjusters, and attorneys; (7) maintain a smoke-free vehicle that is kept safe through regular maintenance and safety checks, including replacement of tires to ensure tread depth greater than the legal minimum; (8) make a visual inspection of my vehicle before leaving on each trip, including a check for proper tire inflation; (9) always have adequate insurance in force; (10) ensure that  the music and conversation inside my vehicle is free of profanity and subject matter inappropriate to the age of the students or to a public school setting (e.g. religious music or conversation).
