Transportation Request
Use this form to request a bus stop change for the following reasons: New Student, Change of Address, or General Request. **NOTE** All requests due to a change of address will require proof of residency to be submitted to the school office that your child is enrolled in. **General Requests will be taken into consideration. All students must have the same bus assignment Monday through Friday. Please refer to Twin Valley School District Policy 810, available at
