2026 Adult Swim ONLY - Members ONLY - 18 and older

(Tell your swim buddies) This form must be filled out ANNUALLY and be on file for all Adult SV CLUB members in order to gain access to the pool via an individually assigned pool gate code. You will receive an email with your "restricted access code" when the form is filled out properly. IF YOU DO NOT FULLY FILL OUT THE FORM YOU WILL GET AN EMAIL ASKING YOU TO DO IT AGAIN. The waiver/statements MUST BE "checked" before you can receive your access code.

It is very important that NO children are in the pool area (not even in the baby pool) during Adult Swim Only. You will be asked to leave and/or your code will be deactivated if children are found in the pool area.

*STAFF MUST BE ONSITE TO ACCESS THE GATE SERVER AND ASSIGN GATE CODES - THERE IS NO OPTION TO REMOTE INTO THE SERVER. IF YOU SIGN THIS FORM ON A WEEKEND IT MAY BE UP TO 48 HOURS BEFORE YOU RECEIVE AN EMAIL WITH A CODE.
