This contract is for parents/guardians that are a part of the Sharks Fastpitch Organization. By signing this contract you are agreeing to follow any and all rules and guidelines set forth by the coaches and managers of the Sharks Fastpitch Organization. You are also acknowledging that if you choose to break any of these rules and/or guidelines you are subject to the punishment that will be issued from coaches and/or managers of the organization.

Do you consent to the release of media (photos, videos, etc.) of your player? This will be used for Sharks Fastpitch Facebook, Instagram, Twitter, FieldLevel, website, etc.  

If you select no, we can not put any pictures of your player on the website, FieldLevel, etc.
