Please submit this form for each student you wish to enroll (1 form submission per student).

Please also ensure the email used, below, matches the Parent email used for our Learning Management System, QuickSchools.

You will receive 2 emails upon successful submission.  The first is an automated invoice from QuickBooks for the Enrollment fees associated with your Enrollment selections.  This should be paid ASAP as it secures your student's place in the class.  Failure to pay the Enrollment fees promptly may result in your student losing their seat in the class as they are first-come, first-served.  The second is an Enrollment confirmation email.

If you do not receive any emails within 24 hours, or you made a mistake / need to make changes, please contact enrollment@faithfcc.com.
