WAIVER OF LIABILITY- (to be signed by Adult Parent/Guardian)I, the undersigned parent/guardian, understand that there may be risks involved with participating in any group activity. In the case where my child has injury, sickness or anything else that may be affected by group activity, I have consulted with my physician to ensure my child can take this class. I recognize that it is my responsibility to notify the instructor of any serious illness or injury before every class. In further consideration of permitting my child to participate in the class(es), I knowingly, voluntarily and expressly waive any claim I may have against Kristi Richardson, Peas and Peaches, the teachers and the hosting facility owners for injury or damages that my child may sustain while on the premises as a result of participating in the class(es). I, my heirs or legal representative irrevocably convenant not to sue and forever release, waive and discharge any other claims of any kind whatsoever against Kristi Richardson, Peas and Peaches and the hosting facility owners and any personal injury, property loss or damage, or wrongful death, whether caused by negligence or otherwise. PHOTOGRAPHS, VIDEOS AND STATEMENTS- I authorize the use of my own and child’s visual image and statements in newsletters, posters and other advertising.
