I hereby give permission for my student to participate in off-campus learning experiences as a part of his or her program at New Vista High School. These activities include but are not limited to: • using the Boulder Public Library;• using resources at the University of Colorado; • attending classes or doing volunteer work at other Boulder Valley schools; • participating in Wednesday Community Experience activities in private, public, and nonprofit agencies throughout the community; • doing research or working at offices and/or natural habitats throughout the community; • taking class field trips along the Front Range. I understand: • that usually, BUT NOT ALWAYS, student learning activities off site will be supervised by either a staff member, students from the University, other volunteers approved by the school, or staff people at community agencies;• that this may involve activities beyond the scope of traditional school functions conducted on School District Property; • that some of these activities potentially involve risks and obligations that are impossible to predict, but may include the risk of loss or damage to personal property and the risk of sickness, personal injury or death; • that the School District does not purchase, or have, any medical, dental or hospitalization insurance to cover injuries to or loss of life of pupils or to indemnify parents and guardians for expenses connected to these risks and that such insurance, if desired, must be purchased by me;• that the activities covered by this permission slip have been approved by the principal of New Vista High School.• that my student will only use one of the following approved modes of transportation to get to and from off-campus learning activities: • walking; • having his/her wheelchair pushed in a safe manner as determined by an adult with expertise and information; • bicycling with a helmet; • taking an RTD or School District bus; • using Special Transit; • driving his/her own car WITHOUT OTHER STUDENTS AS PASSENGERS;• or riding in an insured vehicle, whose driver is a staff member, parent, University volunteer, or an agent of the student’s Community Experience Placement, and who meets the Boulder Valley School District’s insurance requirements. FOR THE MOST PART, STUDENTS WILL GET TO OFF-CAMPUS LEARNING SITES BY THEMSELVES OR IN SMALL GROUPS WITHOUT SUPERVISION. This serves as my electronic signature.
