TO ENSURE YOUR PROPOSAL IS APPROVED,  please take the time to gather ALL information that is required BEFORE submitting this online form. This will include a discussion with your Honors College faculty Internship Advisor (this can be someone other than your Academic Advisor). This proposal is to be completed by you, the student and NOT the internship supervisor. Academic Component information (e.g. number of pages of your written requirement; indicate appropriate readings)

Internship Advisor (must be Honors College faculty/staff)

Internship Supervisor (someone that will oversea your work at the location of the internship; cannot be a family member    

Contact info and title for your onsite internship supervisor

If the internship is UNPAID or PAID (pay status)
   
Number of credit hours

Number of hours to be worked (Remember: total number of hours to be worked per credit hour in the semester depends on pay status.      UNPAID: 40 hours to be worked for every credit hour     PAID: 60 hours to be worked for every credit hour)
