Request to Participate in Competitive Interscholastic Activities, Grades 6-12
As a result of legislation (SB 42-Dexter Mosely Act, O.C.G.A. § 20-2-319.6) effective July 1, 2021, home schooled students in grades 6-12 have the option to participate in sports or other extracurricular activities such as band, drama or school clubs under specific guidelines.  Parents/guardians who desire for their home schooled student to participate in Competitive Interscholastic Activities must complete this request form at least 30 days before the first day of the semester when the child will enroll and participate in the extracurricular activity or sport.  Students who leave the Fannin County School System to enroll in a homeschool program are ineligible to participate in competitive interscholastic activities or sports for the first 12 months after leaving the school system.  For more information, please visit:  https://www.gadoe.org/External-Affairs-and-Policy/Policy/Documents/SB42%20Guidance-Dexter%20Mosely%20Act.pdf

I understand my student must enroll in and attempt to complete at least one class offered by the school in order to participate each semester in an interscholastic activity.  I will schedule an appointment to enroll my child in Fannin County Schools by contacting the Centralized Registrar at (706) 946-2004.  I understand my request for participation cannot move forward until my child is enrolled in the Fannin County School System. *
To enroll in school, I understand that I must present the following information/documents to the centralized registrar:  1.  A copy of my child's birth certificate; 2. A copy of my child's immunization record; 3.  Form 3300 (Eye, Ear and Dental Exam record) available from the Fannin County Health Department (706.632.3023); 4. A copy of my child's social security card; 5.  Two proofs of residency; and 6. A copy of my child's most recent home school grade report from their home school instructor. *
I understand that my child must meet all guidelines and requirements that other students must meet including try outs and attending required practices. *
I understand my child will be subject to the Student Code of Conduct like all other students enrolled in the Fannin County School System. *
I understand that if my student leaves the Fannin County School System to enroll in a homeschool program, he/she is ineligible to participate in competitive interscholastic activities for 12 months.  This timeline is part of the Dexter Mosely Act. *
