Expression of Interest for On-Campus Flu Vaccine: Saturday, October 22, 2022
The Potomac School is currently working with CVS Pharmacy in McLean, VA to schedule an on-campus flu vaccine clinic for our K-12 students and their family members on Saturday, October 22, 2022 from 10:00 am-2:00 pm. If you are interested in participating, please complete the form below so we can gauge interest and predict the number of vaccines needed. Please complete one form per family by Monday, October 10.

CVS Pharmacy will require health insurance information, a completed Vaccine Intake Consent form, and a parent/guardian to be present with a minor child.
