4 square, disc golf, baseball, basketball etc (children will pick with staff guidance)

Due to the max limit, please fill out one form per child.  If you are signing up multiple children you must do multiple forms.  Payment for multiple children can be sent in as one payment. Make sure you back out and click the correct link if you are signing up multiple children.  The forms are for grades 2-7.

Release of Liability: In consideration of permitting the above named child to participate in games, practices and other activities of the Youngstown Recreation Program, I. the undersigned as parent or guardian of said minor, do hereby, release and agree to hold harmless the Youngstown Recreation Program and its said agents, employees and coaches from any liability for personal injury or property damage which may occur to said minor as a result of his/her participation or as a result of negligence on the part of said program or its agents, employees and coached related to the program.
