Customer agrees to instructions and requirements. I will follow all of the instructions of the Business while on the Premises.I agree not to enter the Premises if I am experiencing symptoms of COVID-19 such as cough, shortness of breath, or fever, have a confirmed or suspected case of COVID-19, or have come in contact in the last 14 days with a person who has been confirmed or suspected of having COVID-19. *
Customer assumes the risk. I am aware of the highly contagious nature of COVID-19 and the risk that I may be exposed to or contract COVID-19 by being on the Premises and engaging in the Activity.I acknowledge that I am voluntarily entering the Premises to engage in the Activity with knowledge of the danger involved. I hereby agree to accept and assume all risks of personal injury, illness, disability, or death related to COVID-19, arising from my being on the Premises or engaging in the Activity, whether caused by negligence of the Business or otherwise. *
Customer consent. In exchange for being permitted to enter the premises of The Lash Frontier (the "The Lash Frontier"), located at 3105 Lakeshore Drive Building B #105 (the "Premises") to receive esthetics services and/or attend a Lash Education Course (the "Activity"), I agree to pay for services and/or a Lash Education Course upon completion or prior to the service and/or Lash Education Course and to the following as described in this contract. *
Customer releases seller from liability. I hereby expressly waive and release any and all claims, now known or hereafter known, against the Business and its owners, employees, affiliates, and officers, on account of injury, illness, disability, or death arising out of or attributable to my being on the Premises or engaging in the Activity and being exposed to or contracting COVID-19, whether arising out of the negligence of the Business, its owners, employees, affiliates, officers, or otherwise. *
Completion of Services. The client agrees to pay for the total amount of the service upon completion. The client agrees to accept or decline receipt after payments are made. *
Terms & Conditions. The Lash Frontier does not offer refunds or exchanges. The Lash Frontier is not liable for allergies or injury during or after services. The Lash Frontier has a 24 hour cancellation policy. Rescheduling or cancelling within 24 hours of an appointment will be considered a no-show. No Show clients will be required to pay 100% of the scheduled appointment before being able to book again. Lash appointments more than 4 weeks apart will be considered a new set. Please allow 24 hours prior to appointments for scheduling and rescheduling. Lash retention is contingent upon the care and keeping of your lashes. The Lash Frontier is not responsible for poor lash retention after the first 48 hours of application.* All new and existing clients will be asked to book online in order to keep track of client records correctly.*so long as all aftercare steps have been taken *
Media Release. The customer agrees to have their photo taken in form of video and/or photograph. The customer agrees to have said photos/videos posted to social media. *

Signatures. This form may be signed electronically or in hard copy. If signed in hard copy, it must be returned to the Business for valid record. Electronic signatures count as original for all purposes. By typing their name as a signature below, the customer agrees to the terms and provisions of this agreement. *
1 am 18 years of age or older. I agree to arrive to 3105 Lakeshore Drive B105 at my scheduled time. I agree to lay on my back for 4+ hours with my eyes closed. (Bathroom Breaks Provided) I agree to receiving a full set of eyelash extensions from a potentially uncertified lash artist. I have completed the form above agreeing to instructions and requirements, assumption of risk, consent, release of liability, completion of services, terms and conditions, and media release. *
