Release and Liability Waiver for Vivacity Baton and Dance Club

Vivacity Baton and Dance Parents, legal guardians, staff and volunteers are required to agree to this release and waiver of liability. 

I recognize and acknowledge that there are certain risks of physical injury to participants in the classes that Vivacity Baton and Dance offers and the fundraising activities that this club holds.

Some participants may find certain exercises unsuitable for participation, and any exercise performed incorrectly may involve a risk of injury. I voluntarily agree to assume the full risk of any and all injuries, damages, or loss, regardless of severity, that I may sustain as a result of  participation.

I do hereby fully release and forever discharge Vivacity Baton and Dance the directors, agents, coaches and volunteers from any and all claims for injuries, damages or loss that I may have or which may accrue to me, arising out of, in connection with, or in any way associated with these classes and/or special events that Vivacity Baton and Dance operates.

This Release and Liability Waiver shall be binding upon and inure to the benefit of the parties and their heirs, successors, assigns and personal representatives.
