Student registration form
By completing and submitting the Registration form below, you (the signatory) are agreeing to comply with the following regulations: THIS IS A LEGALLY BINDING CONTRACT BETWEEN THE SIGNATORY AND ANDRIA ANTONIOU.

1. This Registration is valid from the date received for an indefinite period or until the signatory chooses to give the appropriate notice to end their lessons.

2. By submitting this registration form, the signatory will be liable for lessons booked for a minimum of 1 month (4 lessons), though they may terminate their Registration in the appropriate manner at any point after the first 2 weeks of lessons.

3. Lesson payment in accordance with your invoice should be received before the first lesson and no later than 7 days after.

4. Cancellation policy: If the lesson is cancelled in less than 24 hours it is chargeable and not refundable. All lessons cancelled should be then rearranged within the same term (period of three months).

5. In case the lesson falls on a Monday bank holiday (except half term, Easter and Christmas holidays), it should be rearranged on another day.

6. If the student or teacher is experiencing symptoms of cold or flu, the lesson should be done online instead of face to face. If the student/teacher’s voice is affected due to illness, then the lesson will be rearranged on another day (online or face to face). 
Should the signatory wish to terminate your registration, a minimum of 4 weeks notice in writing is required.

7. Payments may be made through online transfer (pay as you go) or prepaid set of lessons or in cash.

8. If the lesson is rearranged, or the teacher is touring abroad, or student/teacher is feeling unwell or for whatever other reason the teacher cannot travel to student’s house, the lesson will take place online.

ONLINE TEACHING
In line with safeguarding procedures for online lessons, I am asking all registered students/parents to agree to the online code of conduct. This is in place to keep everyone safe and working in a positive environment.
Before joining an online lesson, the student is aware he/she must be appropriately dressed, ready at the scheduled time and other people in the house are aware they may be seen or heard *
I agree to not reveal any passwords or links to online lessons to anyone else *
Once a month at the most I send out a newsletter with my latest news on concerts, workshops, singing & movement tips, song releases and anything that I may find inspiring and worth sharing with you! *
