Chillicothe Jaycees & Evening Lions Halloween Parade Entry Form
2017 Halloween Parade – Saturday, October 28th – 7:00 p.m.

Rules:
1. All entries must have a decorated vehicle and/or costumed driver & marching units. Bands and color guards are exempt.
2. Do not use vehicle headlights. Use only parking/running lights and interior lights.
3. Candy throwing is not permitted for the safety of spectators. Candy must be handed out at the curb. –If candy is thrown then you may not be allowed in next year’s parade.
4. Allow two car lengths between vehicles
5. Car club entries are limited to five (5) cars per entry. Clubs may have more than one entry.
6. Political entries are welcome. There is a $50 pre-paid fee per candidate/entry. No flyers, handbills, stickers, or political brochures may be distributed.
7. Business/Commercial entries are welcome. There is a $50 pre-paid fee for the first vehicle, and a $5 fee for each additional vehicle.
8. Entry forms must be received by October 18, 2017, to guarantee entry in the parade.
9. The parade line-up will be posted online at www.chillicothejaycees.org by Friday, October 27th. The parade lineup will also be available on WKKJ website and “Chillicothe Jaycees” Facebook page. You will be able to determine your position in the parade, and where you will line up. Line-up begins at 5:30 pm the day of the parade.
10. No individual bicycles or mopeds allowed. Only antique bicycles and bicycle clubs are permitted.
11. No motorcycles except “full dress” style or fully-decorated are allowed.
12. Horse units are invited. All horses will be located at the end of the parade. Entries must provide manure scooper service.
13. Due to public safety, semi-trucks (tractor trailers) will not be permitted in any entry.
14. See below disclaimers of liability, waivers, and releases that by submission of this entry form and the groups participation in the parade, my group, including but not limited to each and every individual member, agrees to be bound by. You MUST agree to the waiver provided at the conclusion of this entry form in order for the submission to be valid.

ANY ENTRY ACTING IN AN UNSAFE OR ILLEGAL MANNER WILL BE DISCHARGED FROM THE PARADE AND ARE SUBJECT TO PROSECUTION.

Float Judging:
Judging will take place in the Chillicothe City Pool parking log in Yoctangee Park at 5:00 pm. After judging, floats may move to line-up.

Costume Contest:
Judging will take place in front of the main stage (Paint Street, in front of courthouse) at 6:00 pm. Entry forms for costume contest are not necessary—just be present for judging. Children under 12 must be accompanied by an adult.

Disclaimer and Waiver of Liability: The Group submitting this form, including, but not limited to, all individuals participating in the parade with this Group (hereinafter the “Group”), hereby acknowledges and understands that the Group’s participation is purely and entirely voluntary, and that there are certain substantial and inherent risks, known and unknown, involved in the Group’s participation and in the parade which the Group is participating (hereinafter the “Parade”), including without limitation the risk of personal injury and/or death to persons or injury to property. The Group agrees to assume such risks and agrees to release, waive, and discharge the Chillicothe Jaycees, and/or their affiliates, subsidiaries, parent corporations, officers, members, directors, employees, personnel, managers, attorneys, insurers, agents, and volunteers (hereinafter the “Releasees”) from any and all claims, causes of action, demands, lawsuits in law or in equity, arising from, related to, or regarding the Groups participation in the Parade, including without limitation, any claims for death, loss of consortium, bodily injury, or property damage the Group or any individual therein may encounter or sustain as the result of such participation in the Parade, consequential and/or punitive damages, damages for loss of profits, and/or any conduct of the Releasees, whether negligent or otherwise. Likewise, the Releasees disclaim any and all liability for any and all injuries to persons or property that are a result of, pertain to, relate to, or are affiliated with the Group’s participation in the Parade. Indemnification: Further, the Group agrees to indemnify, defend, and hold harmless, to the fullest extent permitted by law, the Releasees from any claims, lawsuits in law or equity, causes of actions, demands, related to, regarding, or pertaining to, the Groups participation in the Parade, including, but without limitation, any claims for death, loss of consortium, bodily injury, or property damage, the Group, or any other individual may encounter or sustain as the result of such participation in the Parade. Photos: I authorize the Chillicothe Jaycees to use the Group/individuals photo, name, likeness, and comments in any form of media including, photos, newspapers, radio, social media, circular or brochure publications, and any other reasonable form of advertising and promotional material. Fitness Acknowledgement: The Group acknowledges that Participation in the Parade may require strenuous exercise. Authority I further state that I have the legal authority to execute this form on behalf of the Group herein named, that I have carefully read the above Waiver & Release and understand the contents of the Waiver & Release and that I am signing this Waiver & Release voluntarily and under my own free will and not based on any representations made to me by any Releasee.
