Your child care center participates in the Child and Adult Care Food Program and receives assistance to serve nutritious meals. We need you to complete or update your forms now. This form is for three children, complete an additional form if you need to add more children.

Please call the Child Nutrition Program office if you have questions at 704-375-3938. Your email address is your digital ID, which verifies your identity and allows for the digital signature.  If you do  not have an email, call and we will mail you a form to complete and sign.
