Use this form to request personal quantities of sexual health supplies, such as condoms and lubricant. These will be discreetly sent to you at your college via the CUSU-MS, for which we require a FULL name and college. Please be assured that this is a completely confidential service.

As a student, you are able to receive 12 condoms (of any type) as well as additional supplies such as lubricant or pregnancy tests.

Please allow up to 3 working days for supplies to reach you. This is to cater for CUSU staff time in assembling your request and the mail service period. If you cannot wait this length of time, speak with your college welfare officer or come into the CUSU office (17 Mill Lane) for immediate supplies.
