SIT Referral Form

The vision of the FLES School Intervention Team (SIT) is to collaborate around student needs to ensure that all students will perform at high levels.

Utilize this form when additional support or ideas about student behavior or academics is necessary. After you have implemented the plan that was created with colleagues AND parental input.

The SIT Team meets biweekly on Tuesdays and will review, share ideas, and provide feedback to you as soon as possible.

If submitting an urgent concern, administration may reach out prior to SIT meetings to address needs in a timely fashion.
