Play Up/Play Down Request Form - DLL

Please fill out this form if you wish for your player to move up or down a division for the 2024 spring season. The DLL Play Up Committee will consider division change requests for the following reasons:

1. Your child has played in their current division for two years.

2. There is perceived or potential for increased injury risk due to the player's level of play.

All other reasons are reviewed on an individual basis and are only granted at the discretion of the Board of Directors.

You must complete this form by March 1, 2025 to be eligible.
