Dear Student,  This form is only to be used for short term sickness. If you have a GNIB appointment, long term sickness (greater than 2 consecutive days) or any other non-health matter, which might affect your attendance, please contact the Citas Team at info@citas.ie or come in and see us.  You can be excused from classes due to a short illness for 2 consecutive days without a Doctor’s note, and for a maximum of six days uncertified over the 25 weeks of your course.  Please can you fill in all parts of the form below so that we can process your information quickly and effectively.  This form must be submitted every day you are sick by 12pm at the latest.
