WAIVER FORM FOR CHURCH SUNDAY SCHOOL (KIDS) MINISTRY

I, [Parent/Guardian’s Name], hereby give permission for my child, [Child’s Name], to participate in the Sunday School Ministry at Living Grace Church. I understand that the Sunday School Ministry includes various activities such as games, lessons, and other related events.

I acknowledge that participation in the Sunday School Ministry involves certain risks, such as accidents, injuries, or illnesses that may result from my child’s participation. I hereby assume all such risks on behalf of my child and myself, and I release the Living Grace Church, its pastors, officers, employees, and agents from any liability, claims, or demands that may arise from any injuries, losses, or damages that may occur during my child’s participation in the Sunday School Ministry.

I also agree to indemnify and hold harmless the Living Grace Church, its pastors, officers, employees, and agents from any liability, claims, or demands that may arise from any injuries, losses, or damages that my child or I may cause to others or their property during my child’s participation in the Sunday School Ministry.

I further acknowledge that the Living Grace Church has the right to use photographs, videos, and other recordings of my child for promotional purposes related to the Sunday School Ministry and extracurricular events.

I have read and understand this waiver form, and I agree to its terms and conditions. I understand that my child’s participation in the Sunday School Ministry is voluntary and that I may withdraw my child’s participation at any time.
