Photoshoot request + deposit - fall/winter 2025
Please fill out this form in order to secure your spot and submit your deposit for a photoshoot with HHS student journalists and adviser Hailey Phillips. Deposit of $10 must be made to ensure your spot is secured. Deposits MUST be made within 24 hours of submission of this form to Mrs. Phillips through cash OR checks made to Humboldt High School to officially secure your spot and photoshoot time. Failure to do this within the 24-hour window will forfeit your spot automatically. We have a limited number of slots available. 

Once you fill out this form and pay, Hailey Phillips will reach out with photoshoot times and location confirmation. First come first served. 

**Keep in mind we will have additional shoots in the spring time if you are unable to get in this time!**

1. Desired date of photoshoot (Nov. 16  or Dec. 7) 
2. Preferred times that you can appear for a 30 minute shoot between 3 and 6 p.m. on one of those dates.

I agree to pay my deposit ($10 due at time of scheduling to secure spot) and full payment of a total of $25 due at beginning of shoot.
