ABILITY TO ENGAGE IN ART ACTIVITIES:  Creative Camps takes precautions to reduce risk and provide a safe and healthy environment and an enjoyable experience for campers. I warrant that my child is able to follow directions for all activities during programs. I acknowledge that risks from participation in Creative Camps programs exist and that I have allowed my child to attend Creative Camps knowing these risks and their possible consequences including personal injury.   

WAIVER AND RELEASE OF LIABILITY: As a parent or guardian of my child, I agree that I will not hold Creative Camps NC, LLC liable for any personal injury or property damage. I agree to release and hold harmless Creative Camps NC, LLC and owner Megan O'Connor Upchurch from all liability incurred as a result of my child's participation in Creative Camps. By clicking 'I agree' I hereby waive and release all claims, medical bills, and liabilities of any kind or nature.
