Official UOO Opt-Out/Refusal Survey
We at United Opt Out (UOO) are collecting information on refusing or opting out of testing. Completion of this survey will enter you into a drawing to win an autographed copy of our official poster for Occupy2.0: The Battle for Public Schools. The deadline to complete this survey will be June 15th, 2013.

It is becoming increasingly important to collect information to assess the success and progress of the movement to resist high-stakes, standardized testing. Being able to cite basic numbers about parents, teachers, and others involved in an opt-out movement is essential to gaining momentum and credibility. Your participation in this survey is entirely voluntary and we appreciate your consideration.

All of your information will be kept confidential and private; no identifying details will be shared. However, the more information you can provide, the better our assessments can be.

By continuing with this survey, you consent to the use of this information for analytical purposes. Your name or any other personal information will never be shared. Should you include your email address, which is entirely optional, someone at UOO may contact you via email to ask potential follow-up questions (We also need a way to get in touch with you should you win the poster!).
