This form is to be completed by any candidate wishing to apply for a position on the Welsh Boxing High Performance Coaching Cohort for the 2027-2028 cycle.
Candidates must complete ALL questions.
Any incomplete submissions will not be accepted or reviewed, so it may be advisable to copy and paste your responses on an additional document, should you need to return to complete the submission.
If you are unable to complete this form, you can submit a comprehensive video submission of your application, clearly stating the questions and your answers. Each Question will need to be submitted via video. Any incomplete video submissions will not be considered.
