UCA Facility Usage Request Form & Contract
All information must be filled out in order to process your application. Your copy of your contract will come to you in an email format with all of your responses. Please keep this email for your records. After completing the form, you will be redirected to a copy of the Facility Usage General Policies so that you can print them if you would like them for your records.

Everyone reserving a space must submit this form to UCA Conference Services at least 2 weeks prior to the requested date. All required forms must be completed and submitted before a reservation can be confirmed.

Event Set-up *
Detailed set-up requirements for your event MUST be included or your event will not be placed on the calendar. This is for informational purposes only, THIS IS NOT A WORK ORDER. Physical Plant work order may be required for your event. Set-up examples are theater style (chairs only), rounds (banquet style), classroom (long tables/chairs) and u-shape.
