This is the registration form for the AdG "Dance with Us" Series African Dance Class. Please provide your information for proper registration of the class.

Please note: exercise is not without its risks and this or any other dance class or exercise program many result in injury.  Please refrain from signing up for class if your physician or health care provider advises against it, or if you have a medical or physical condition that may increase your risk of injury or death. If you experience faintness, dizziness, pain or shortness of breath at any time while exercising you should stop immediately. Those experiencing the COVID symptoms, please do not sign up for this class at this time. Those taking our african dance class do so at their own risk.

Within the date of your registration and the time of class if you have been in contact with who has tested positive for COVID19 or exhibiting any symptoms please let us know and understand that contact forfeits your attendance to the class. If you have to forfeit class your payment will be refunded.

***IMPORTANT***: If you are fully vaccinated, please provide proof of your vaccination card when you arrive to class and mask will not be required. Masks are REQUIRED for unvaccinated participants. Thank you for keeping us safe!
