- First names only (or a nickname if you go by one).
- You can enter up to 4 additional passengers. For each additional person please include Name, Relationship to you, and Age at time of cruise (more for those under 18).
- If you have more than 1 stateroom, please fill out separate form for each room but remember to put your DISname in the first box.

*Just a reminder that while the Roll Call spreadsheet is a fun way for us to get to know each other and see who's part of our group before the cruise, it is not private. The spreadsheet is accessible to anyone who reads our thread. While we are asking for first names only, if you do not want your first name on the sheet you can enter a nickname or no name at all. And for any other passengers traveling with you it's ok to be vague and enter something like DH or DS(12). Same goes for your Stateroom number. It's ok to enter GTY or your Deck number instead of your actual Stateroom.

If you make a mistake and need to change something or if you want to remove your name you can change your info by clicking on the "See Who's Cruisin' - Alaska May 24, 2014" link on the first page of the DIS thread and make the necessary changes to your info. You do not need to contact anyone edit your own info.
