2026/27 SMHA  Excused Absence Request Form

SMHA expects all players participating in the Competitive Tryout process to make every effort to attend all scheduled sessions. However, SMHA recognizes that, in certain circumstances, a player may be unable to participate through no fault of their own. Excused Absence requests are reviewed on a case-by-case basis and approval is not guaranteed.

Eligible circumstances may include:

  • Illness or injury, supported by a medical note from a qualified medical professional. The anticipated return to full on-ice participation, including games, must be no later than December 1.
  • Compassionate circumstances, such as a family tragedy.
  • Provincial or national-level sporting competitions arising from an established player commitment.
  • Specific religious holidays.

Parents/Guardians should submit the request through the Excused Absence Request Form as soon as the absence is known and provide as much detail and supporting documentation as possible, including the expected departure and return-to-play dates.

Requests will be reviewed by SMHA representatives and will be designated as either an Excused Absence or Unexcused Absence. Absences for school trips, other sports or extracurricular activities, family gatherings, or similar commitments will generally be considered unexcused, except where specifically permitted under SMHA policy.

For approved illness or injury absences, a medical note confirming clearance to return to SMHA-sanctioned hockey must be provided before the player resumes participation. Players with an approved Excused Absence will be placed at the conclusion of the Competitive Tryout process; no placement decisions are made before or during the process, and placement decisions are final.
