FCSS Online Learning (OLL) K-8 Withdrawal Request
We understand that circumstances change rapidly during this time, and you may have decided that it was best to send your student(s) for face-to-face learning on school campus. Please list all students you would like to withdraw from online learning. By completing this form, we will withdraw your student(s) from the online learning platform, and he/she/they will be re-assigned for face-to-face instruction. You can list up to four students on this form. Your child is expected to be in attendance for face-to-face instruction at his or her home school the day after you complete this withdrawal request.

I acknowledge that my child is being withdrawn from Online Learning (OLL) at my request.  I understand that if I desire to place my child back in OLL at some point in the future, this will require a new physician’s note and a new OLL request form submission.  I understand that if I have already provided a physician’s note to the school system for my child to enter the OLL program, that note will become null and void upon submission of this withdrawal form.  I further assert that my child is medically able to return to face to face instruction.   (Please type parent/guardian name as your signature.) *
Your child is expected to be in attendance for face-to-face instruction at his or her home school the next school day after you complete this withdrawal request. After you submit this request, you do not need to do anything additional to transfer your child back to face-to-face instruction. The school will begin taking attendance for students on the day following the submission of this form, so if they are not at school on the next day, they will be marked absent. *
