Our mission in Faith Engagement is to nurture the open minds, loving hearts, and helping hands of our children and youth so they may embody our Unitarian Universalist Principles.

Please complete the form below to register your student(s) (birth - 12th grade) for our Faith Engagement programs. This information will be kept confidential.  It will be used to communicate with you and to ensure we provide a caring, safe environment for all participants.

Please note that upon completion of this Participation Form, your child(ren) are PERMANENTLY REGISTERED for UUCWC Faith Engagement Programs (current and future program years).  You will not need to complete this form again unless you have changes or updates to your pertinent information.

UUCWC's Our Whole Lives and Coming of Age programs require a separate application form available upon request or on the website.

If at any time you would like to be OFFICIALLY REMOVED from our program roster, please contact the Director of Family Ministry at sahrens@uucwc.org with your request.
