FOR INSTRUCTORS & TRAINERS:

1) Click "USE TEMPLATE" at the top-right of the window. This will open a new form with this template, belonging to your Google account. You may wish to move this form now, to a place in your Google Drive that will make it easier to find later (click on the folder icon near the top-left of the window to select a location).
2) Make sure the template is closed and you are now working in your own copy -- please do not change or send out the original template. :)
3) Select or add one question you would like to ask for your "Green" and one for your "Red" sticky note. Delete the others.
4) To get a link for your learners to use, click the "Send" button at the top right, then select the link tab in the window to get a URL.
5) Delete these instructions from your copy
