CHARACTER CARNIVAL PARTICIPATION/TRAIT SELECTION FORM
Calling all students, clubs and groups! We are so excited for the Carnival this year, on March 4th, but we need your help.

The main focus of the Character Carnival is the wonderful and creative booths sponsored by clubs, groups, students and teachers in our schools and community. If you would like to participate, below is a list of traits that are available for you to pick from. Please choose two-three in case one has been taken. We will let you know within a few days what virtue you have. They will be assigned in the order they are requested. If you have several students and/or helpers and would like to have two character trait booths, please indicate that below.

We appreciate your enthusiasm and cooperation in helping us put on a premier event for the WNY community!

The Carnival will be held at the HUB 4999 McKinley Pkwy Hamburg, NY 14075 on March 4th from 10AM-3PM.

In addition to your incredible booths, we will have lots of family friendly entertainment to round out the day!
