The Northville Public School District requires that a criminal background check via the Internet Criminal History Access Tool (ICHAT) with the Michigan State Police be conducted for individuals who school officials reasonably expect may have regular and continuous contact with students, or may at times be with students absent the presence of a school staff member.  This may include but is not limited to:  pre-student teachers, field trip chaperones, regular volunteers, extra-curricular group leaders and anyone handling cash or district funds.

I understand that the requested information is required by the Central Records Division of the Michigan State Police or Northville Public Schools for the purpose of conducting a criminal background check through the Internet Criminal History Access Tool.

I hereby authorize the Northville Public School District to utilize this information for the sole purpose of conducting a criminal background check through the Internet Criminal History Access Tool.

I understand that this form and the results of the above mentioned background check shall be the property of the Northville Public School District.
