OU Math Day 2019 Registration Form
Math Day this year will take place November 7th at the Thurman J White Forum Building on our Norman Campus.

To register your students, please fill out the form below.
- Each school only needs to fill out this form ONCE.
- You can change your answers to this form at you fill it out (a link will be emailed to the address provided).
- Registration will close when we are at capacity, or on November 3rd.
Please estimate the number of students you will bring.
You may always change your answer later.
Individual exam participants
One of the events in Math Day consists of an individual, written exam for each student. They are simultaneous, so each student should sign up for one exam. We encourage students to take the exam for the most recently finished course. Please enter approximate numbers if you are unsure.
Chaperones and Policies
Policy on chaperones
University policy states that there must be at least one chaperone (teacher, parent, etc.) for every 12 students. Chaperones will be provided with lunch at no cost.

Policy on payment
The cost per student is $10. Please bring a check or payment order with you on the day of the event. (Do not mail a check, and please do not bring cash.) Make checks payable to OU Foundation #31247. The Tax ID Number (TIN) is 73-6091755.
