CDF Wish Recipient Nomination Form
The Cloud Dancers Foundation is a registered 501(c)(3) with a mission to grant wishes to those who feel invisible, starting with the mature transgender community, inspired by our founder, Robina Asti.

We are currently accepting nominations for U.S. residents who are transgender and 35+ years of age.

Please submit a nomination below for yourself or someone else. Note that filling out this form is not confirmation of eligibility for a wish. The Cloud Dancers Foundation will be in touch if your nominee is eligible.
