We're thrilled to have you join us at the 24th Annual JCC Sports Dinner on Wednesday, March 8th, 2017 at the Hyatt Regency Hotel. Please complete and submit this form to reserve your table. Each table seats 10 people.

Please note that your completed table list will be required two weeks prior to the event. A friendly reminder that there are a limited number of tables available and they will be reserved on a first-come, first-served basis (we did sell out early last year). Table purchases include GST and are non-refundable.

Would you like a tax receipt?
A tax receipt for the donation portion of the table purchase is $1,480. The tax receipt will be mailed to the company or name at the address listed above.
You will receive an email one month prior to the event with the table list form. We require it fully completed and returned to our office 2 weeks prior to the event (February 22nd). Important event information will also be provided to you then.
