Fine Arts Center Reservation Form
If you are requesting use of the Mountain Brook High School Fine Arts Facility, please complete this formal request. To fully complete this form, you will need the dates of your request and information about other equipment you will need provided by MBHS.

Please turn in the form at least FOUR (4) weeks prior to the requested event date(s). Of note, not all requests can be honored and MBHS School requests are honored before outside use.

If dates are flexible, enter additional dates in requested order. A representative will email you to confirm the dates and ask any additional questions.
