We are so thankful for your interest in joining the Shaw Spouses' Club this year! Once we receive this completed form and your dues, you will be added as a member to our club!

By submitting this form, you agree to the following:

- All reservations and cancellations must be received by the RSVP deadline
- No shows will be charged for the event unless another member/guest can take their place
- Socials are for adults only, unless otherwise stated
- See Constitution for information about approved guests
- Special Activity Group are for SSC members (adults) only unless otherwise specified by the group leader
- SSC Members agree to abide by the SSC governance posted on our website (www.shawspousesclub.com) and/or available upon request
- All members of the group are jointly and severely liable for the obligation of the PO
- The Shaw Spouses' Club is a private organization. It is not part of the Department of Defense or any of its components and has no governmental status
