If you're a Club/Organization or Resident Assistant and would like to host a Fitness class for an event please complete the following form. Please remember that in order to find the instructor the Fitness & Wellness Staff will need at least 2 weeks time. You will be invoiced for the Instructors time which can be paid either cash or check after the event occurs.

The invoice will come directly from The Department of Campus Recreation and is due upon receipt. The invoice is payable by cash or check and needs to be paid regardless of attendance to event. *
